In a Nutshell…

  • CHARTSandNOTES is free to adopt, to use, and to maintain.
  • Stop using it any time: no lock-in. Even use it only part of the time, if you prefer.
  • It’s independent of internet, cellphone, and cable.
  • Take it with you: it’s entirely self-contained on your laptop or desk computer.
  • Just add records to your existing paper charts: no need for conversion.
  • Your own reminder notes are always at hand, for diagnoses, therapies, or investigations.
  • Records are all encrypted and stored instantly on your computer and/or flash drive.
  • Example: records for 10,000 patients with 10 visits each are stored in 1.4GB.

Definitions: Charts and Notes

  • CHART in the present context means the physical folder containing all the hand-written and printed records related to a single patient. A typical solo general practitioner’s office may contain thousands of individual charts generated over the years.
  • NOTE here means a reminder of general information, which would not be stored in any one patient’s chart. These notes can be recorded while looking up data in books or journals, and then will be instantly available anytime later. Make and keep notes on drugs, diagnostic tests & images, or diagnoses.
  • CHARTSandNOTES is a software application that allows the user to generate and store both charts and notes on a single PC or laptop computer, for instant retrieval at any time, without requiring any internet or telephonic connection.
